ABSTRACT

Background: central nervous system (CNS) hyperperfusion is one of the events that constitute the pathophysiological basis for the clinical manifestations and complications of pre-eclampsia (PE). Detecting the increased flow in the CNS through Doppler flowmetry of the ophthalmic artery might precede the clinical onset of PE and could be used as a marker for subsequent development of PE. Objective: to evaluate the ophthalmic artery resistive index (OARI) values in the second trimester of pregnancy for prediction of the clinical manifestations of PE. Objective: to evaluate the ophthalmic artery resistive index (OARI) values in the second trimester of pregnancy for prediction of the clinical manifestations of PE. Methods: a total of 73 patients with risk factors for the development of PE were selected from the prenatal service at the HC-UFMG. They were submitted to ophthalmic artery Doppler flowmetry between 24 and 28 weeks of pregnancy and monitored until the end of the pregnancy to verify the occurrence of PE. ROC curves were created to determine the predictive characteristics of the OARI. Results: fourteen of the patients selected developed PE and 59 remained normotensive until the postpartum period. Patients with subsequent development of PE presented OARI values lower than patients that remained normotensive (0.682±0.028 X 0.700±0.029, p=0.044). Considering the development of PE as an outcome, the area under the OARI curve was 0.694 (CI 0.543 to 0.845), with no points obtaining good values of sensitivity or specificity. Conclusion: Doppler flowmetry of ophthalmic arteries between 24 and 28 weeks of pregnancy did not present itself as a good exam for predicting PE. .


Introdução: a hiperperfusão do sistema nervoso central (SNC) é um dos eventos que constitui substrato fisiopatológico para as manifestações clínicas e complicações da pré-eclâmpsia (PE). O fluxo aumentado no SNC, detectado por meio da dopplerfluxometria de artérias oftálmicas, poderia anteceder as manifestações clínicas da PE e, consequentemente, ser utilizado como marcador de subsequente desenvolvimento de PE. Objectivo: avaliar os valores do índice de resistência das artérias oftálmicas (Irao) no segundo trimestre gestacional para a predição das manifestações clínicas da PE. Métodos: pacientes com fatores de risco para desenvolvimento de PE foram selecionadas no serviço de pré-natal do Hospital das Clínicas da Universidade Federal de Minas Gerais (HC-UFMG). Elas foram submetidas à dopplerfluxometria de artérias oftálmicas entre 24 e 28 semanas de gestação e acompanhadas até o final da gestação para averiguar a ocorrência de PE. Curvas ROC foram criadas para determinar as características preditivas do Irao. Resultados: das pacientes selecionadas, 14 desenvolveram PE e 59 mantiveram-se normotensas até o puerpério. Pacientes com subsequente desenvolvimento de PE apresentaram valores de Irao menores do que pacientes que se mantiveram normotensas (0,682±0,028 vs. 0,700±0,029, p=0,044). Ao considerar o desenvolvimento de PE como desfecho, a área sobre a curva do Irao foi de 0,694 (IC 0,543-0,845), sem pontos com bons valores de sensibilidade ou especificidade. Conclusão: a dopplerfluxometria de artérias oftálmicas entre 24 e 28 semanas de gestação não se demonstrou um bom exame para a predição de PE. .

ABSTRACT

PURPOSE: To evaluate orbital blood flow velocities and optic nerve diameter with Doppler and gray-scale sonography in patients with acute unilateral optic neuritis (ON). METHODS: Orbital Doppler and gray-scale sonography was performed in 46 eyes of 23 patients aged 19- to 47-years with acute unilateral ON. ON was diagnosed by an ophthalmologist on the basis of clinical presentation, presence of decreased visual acuity and assessment of visual evoked potentials. The peak systolic velocity (PSV) and end-diastolic velocity (EDV), as well as the resistance index (RI) and pulsatile index (PI) of the ophthalmic artery (OA), central retinal artery (CRA), posterior ciliary arteries (PCAs) and optic nerve diameter were measured in both eyes. We compared results from affected and unaffected eyes using the paired t-test. The area under the receiver operating characteristic (ROC) curves was used to assess the diagnosis of ON based on measured blood flow parameters of the OA, CRA and PCAs and optic nerve diameter. RESULTS: The mean (standard deviation) optic nerve diameter in eyes with ON was 4.1 (0.8) mm, which was significantly larger than the 3.0 (0.4) mm diameter measured in unaffected control eyes (p 0.05). The mean RI in the PCAs was slightly lower in the eyes with ON than in the contralateral eyes (0.60 vs. 0.64, p < 0.05). The area under the ROC curves indicated that optic nerve diameter was the best parameter for the diagnosis of ON. CONCLUSIONS: Optic nerve diameter was related to ON, but orbital blood flow parameters were not.

ABSTRACT

Foram utilizados 20 gatos sem raça definida, com massa corpórea entre 2 e 4 quilogramas, hígidos, anestesiados com tiletamina e zolazepam (6,0 mg/kg). A artéria femoral foi canulada de forma asséptica e um manômetro acoplado a uma coluna de ar foi conectada ao cateter, a PAM foi aferida antes e a cada 15 minutos até o término do exame ultra-sonográfico. A fluxometria da artéria oftálmica externa foi obtida através do equipamento de ultra-sonografia dotado de eco-doppler, com transdutor setorial de 6,0 MHz, aplicado diretamente sobre a córnea. Cada olho teve o fluxo de sua artériaaferido pelo programa do próprio aparelho, sendo tomadas três amostras. O índice de resistência vascular foi calculado com base nos valores do fluxo sistólico e diastólico médio de cada vaso. Obteve-se um valor médio para a PAM de 144,9 ± 26,68 mmHg e uma velocidade de fluxo de 41,3 ± 14,28 cm/s para a artéria oftálmica externa no olho direito em sístole, 23,95 ± 11,46 cm/s em diástole e 42,75 ± 12,64 cm/s no olho esquerdo em sístole, 25,45 ± 9,61 cm/s em diástole, não existindo diferença significativa entre os olhos. O índice de resistência vascular calculado foi de 0,4175 para a artéria oftálmica externa no olho direito e 0,4015 para a do olho esquerdo, sem diferença significativa entre os olhos. Os resultados apontam para um mecanismo intrínseco de controle do fluxo, controlado por fatores locais na artéria oftálmica externa. Os valores do índice de resistência denotam uma artéria de baixo fluxo. Como conclusão, têm-se os valores de referência acima citados para a PAM e para o fluxo sangüíneo na artéria oftálmica externa em gatos sem raça definida anestesiados com tiletamina e zolazepam, e a informação de que não existe correlação entre esses valores.


There were used 20 mixed breed cats, both male and female, with body mass between 2 and 4 kilograms, that received anesthesia with tiletamine and zolazepam (6,0 mg/kg). The femoral artery was assepticale cannulated and a manometer connected to an air column was connected to the catheter. The MAP was surveyed before and every 15 minutes until the end of the ultrasound examination. The flow of the external ophthalmic artery was measured through the ultrasond equipment, with an echo-doppler and a sectorial transducer of 6.0 MHz, directly applied to the cornea. Each eye had the artery flow surveyed by the equipment's own device, 3 samples being taken. The vascular resistance index was calculated based on the mean values of systolic and diastolic flow of each vasses. An average value for the MAP of 144.9 ± 26.68 mmHg and a flow of 14.28 ± 41.3 cm/sec for the external ophthalmic artery in the right eye in systole, 23.95 ± 11.46 cm/sec in diastole and 42.75 ± 12.64 cm/sec in the left eye in systole, 25.45 ± 9.61 cm/sec in diastole, were verified, there being no significant difference between the eyes. The calculated index of vascular resistance was of 0.4175 ± 0.0774 for the external ophthalmic artery in the right eye and 0,4015 ± 0.0719 for the left eye, without any significant difference between the eyes. The results point to an intrinsic mechanism of flow control, controlled by local factors in the external ophthalmic artery. The values of the resistance index denote a low flow artery. Concluding, this study revealed the reference values, above cited, for the MAP and the blood flow in the external ophthalmic artery in mixed breed cats, anesthetized with tiletamine and zolazepam, and the information that there is no correlation between these values.

ABSTRACT

O exame de Doppler colorido foi empregado para analisar os parâmetros hemodinâmicos (velocidade de fluxo e índice de resistência) em artérias oftálmica, central de retina e ciliar posterior curta de 70 indivíduos normais. Os autores observavam valores semelhantes em órbitas direita e esquerda, que näo há diferença estatisticamente significativa entre os valores obtidos na populaçäo masculina e feminina, nem tampouco uma associaçäo entre idade e parâmetros hemodinâmicos. Uma vez conhecidos os valores para uma populaçäo normal, torna-se possível o emprego do Doppler colorido na investigaçäo de condiçöes oculares associadas a desordens vasculares.
